Terms of Service

These terms govern your use of signaturehealthintel.com and your purchase of Signature Health Intelligence (“SHI”) programmes, operated by Dr. D. R. Gamalathge, MBBS (SLMC No. 36999), Dr. Gamalathge Healthcare Centre, Mirihana, Nugegoda, Sri Lanka. By using the site or enrolling in a programme you accept these terms.

1. What SHI is — and is not

SHI is an annual, physician-led preventive health programme. Each programme (Essential, Premium) includes a defined set of laboratory investigations, an in-person consultation, a physician-authored report, and a three-month Delta review, as described on this website.

SHI is not an emergency service. If you have chest pain, difficulty breathing, or any urgent symptom, call 1990 (Suwa Seriya) or go to the nearest hospital immediately.

Website content is not medical advice. The information on this site is general and educational. Browsing the site or sending an enquiry does not create a physician–patient relationship; that relationship begins when you enrol in a programme and attend your consultation. Your SHI report is a clinical document written and signed by Dr. Gamalathge. A result that needs treatment is treated at the results consultation or referred, with a letter, to the appropriate service.

2. Eligibility

Programmes are available to adults aged 18 and over. Investigations and consultations are performed in Sri Lanka.

3. Booking and enrolment

Enquiries are followed by a short phone or WhatsApp conversation to recommend the right tier before any payment. A booking is confirmed once we agree an appointment and payment is made. You agree to provide accurate personal and health information; the quality of your report depends on it.

4. Pricing

Prices are stated in Sri Lankan Rupees on this website. Founding-patient pricing is limited to a stated number of enrolments and may be withdrawn once filled; launch pricing then applies. The Month-12 full repeat is not part of the first programme year; it is a Year-2 renewal, purchased separately at the renewal price published at the time. Founding-patient renewal prices (LKR 35,000 Essential, LKR 65,000 Premium) apply to continuous renewal by founding patients; the standard renewal prices are LKR 49,500 (Essential) and LKR 81,000 (Premium). Physician additions are optional, priced individually (the laboratory’s price plus LKR 500, rounded up to the nearest LKR 100) and agreed in writing before the draw. Follow-on tests (free T4, folate, SHBG) are run only if a first result calls for them, at laboratory cost, and only with your prior written agreement. Both are charged in addition to the programme fee. We may revise prices for future enrolments; the price you accept at booking is the price you pay for that programme year.

5. Rescheduling, cancellation and refunds

You may reschedule an appointment with at least 24 hours’ notice. If you cancel your enrolment at least 48 hours before your sample collection, any programme fee paid is refunded in full. Once sample collection has taken place, laboratory and clinical work begins immediately and fees for that programme year are non-refundable, save where required by law or where we cannot deliver the programme. Optional physician additions not yet performed are refundable.

6. Reports and clinical scope

SHI is a preventive, screening-oriented service. It is not a comprehensive diagnostic work-up, and no screening programme can exclude all disease. Findings depend on the accuracy of the history you provide and on laboratory analysis performed by our partner laboratory. Typical report delivery is about 7 days from sample collection on Essential and about 14 days on Premium; timelines are estimates, not guarantees. Where results require urgent attention, we will contact you without delay.

7. Your responsibilities

Follow pre-test instructions (such as fasting) as advised; attend your consultation; inform us of relevant conditions and medications; and tell us about any other doctor treating you for a condition the report touches.

8. Liability

Nothing in these terms excludes or limits liability that cannot be excluded or limited under the law of Sri Lanka, including in respect of professional negligence. Subject to that, the website is provided “as is”; we are not liable for interruptions, errors, or third-party services linked from the site; and our total aggregate liability arising out of or in connection with the website or a programme is limited to the fees you paid to us in the twelve months preceding the claim. We are not liable for indirect or consequential loss.

9. Intellectual property

The content, design and branding of this website and of SHI reports are the property of Dr. D. R. Gamalathge, trading as Signature Health Intelligence. You may not reproduce them for commercial purposes without written permission. Your report is yours to use for your own care, including sharing it with your treating physicians.

10. Third-party services

WhatsApp, Instagram, Google services and other third-party platforms linked from this site are governed by their own terms and privacy policies, which we do not control.

11. Privacy

Our handling of personal data is described in the Privacy Policy, which forms part of these terms.

12. Changes, law and disputes

We may update these terms; the version published at the time of your enrolment applies to that programme year. These terms are governed by the laws of the Democratic Socialist Republic of Sri Lanka, and the courts of Sri Lanka have exclusive jurisdiction. The practice of medicine by Dr. Gamalathge is regulated by the Sri Lanka Medical Council.
